Every morning, without much thought, milk finds its way into millions of Indian homes. Into the first cup of chai. Into a child's glass. Into curd, paneer, kheer, and a dozen other things that form the quiet foundation of how we eat every day.
Most of us assume it is what it says it is.
It often is not.
What is happening in Maharashtra right now
On May 25, 2026, IAS officer Tukaram Mundhe took charge as Commissioner of Maharashtra's Food and Drug Administration. Within days, he launched what has become one of the most extensive food safety enforcement drives in the state's history.
In the first thirty days alone, the FDA conducted 360 raids, registered 235 FIRs, arrested 350 alleged adulterators, and sealed 274 illegal units operating across Maharashtra. The operation, running under the campaign name "Safe Food, Safe Maharashtra," exposed an organised synthetic milk network spanning Pune, Ahilyanagar, Solapur, Jalna, and Thane. Investigators found that the syndicate had been manufacturing artificial milk using nearly 2.3 lakh kilograms of low quality milk powder, mixed with genuine milk in a 10 percent ratio before being supplied to consumers.
Mr. Mundhe's approach was different from routine enforcement. Instead of collecting samples from markets and waiting for test results, the FDA inspected the entire supply chain including collection centres, dairies, transportation networks, distributors, wholesalers, and retailers. No one was exempt. When the iconic K. Rustom ice cream parlour in Mumbai's Churchgate was found to have serious hygiene violations including live rats, expired flavouring agents, and broken cold chain practices, its licence was suspended. Heritage was not an excuse.
His own words make the intent plain: "Milk is not merely a food product. It is a nutritional foundation for millions of children, mothers, patients, and senior citizens. Adulterating milk amounts to playing with public health."
He is right. And from where we sit as clinicians working with patients managing diabetes, metabolic syndrome, and obesity, this is not just a food safety story. It is a metabolic health story.
What is actually being added to milk
The list of adulterants found across India and now being actively targeted in Maharashtra is not abstract. These are chemicals with known health consequences.
Water is the most common adulterant, and while water itself is harmless, the water added to milk is often contaminated, carrying bacteria, pesticides, and pathogens that cause gastrointestinal illness and, over time, chronic gut inflammation.
Detergents are added to create a frothy appearance that mimics full fat milk. Detergents contain sodium lauryl sulphate, phosphates, and dioxane. Research published in ScienceDirect found that excessive consumption of detergents and surfactants is linked to intestinal inflammation and bacterial translocation across the intestinal lining. These conditions are associated with increased risk of coronary artery disease and Type 2 diabetes.
Urea is added to artificially inflate protein readings, since nitrogen content is used as a proxy for protein in standard tests. Urea overburdens the kidneys, which must filter out the excess nitrogen load. For someone already managing diabetic kidney disease, even low level chronic urea exposure through food is not a trivial concern.
Starch is used to thicken diluted milk. Research published in the Journal of Medical Evidence noted that high starch content in adulterated milk causes gastrointestinal problems and poses particular risk for people with diabetes, in whom undigested starch in the colon worsens blood sugar instability.
Hydrogen peroxide is added to extend shelf life. It damages gastrointestinal cells, causes gastritis, and promotes intestinal inflammation. Formalin, another preservative adulterant, is a recognised carcinogen that causes nausea, abdominal pain, and at higher exposures, organ damage. Melamine, familiar from the 2008 China scandal, is added to falsely elevate protein readings and causes severe kidney damage.
Oxytocin, the hormone injected into cattle to force higher milk yield, transfers in small quantities into the milk itself. Its effects when consumed chronically through food, particularly on the hormonal and endocrine systems of children and women, remain an active area of concern in Indian research.
The metabolic health connection
This is where the story stops being abstract and starts being directly relevant to anyone who cares about what goes into their body every day.
The gut is not a passive tube. It is an active metabolic organ, home to trillions of bacteria that regulate inflammation, insulin sensitivity, appetite hormones, and immune function. Chronic low level exposure to gut irritants including detergents, peroxides, contaminated water, and formaldehyde does not produce a dramatic illness. It produces something more insidious: persistent low grade intestinal inflammation, disruption of the gut microbiome, and a gradual worsening of the metabolic environment.
Research has now established clearly that gut inflammation and microbiome disruption contribute directly to insulin resistance. The same inflammatory signals that damage the intestinal lining also impair insulin signalling in muscle and fat cells.
Milk is not a peripheral part of the Indian diet. It is central to it, consumed daily, by every age group, in multiple forms. If what is arriving in homes is not milk but a chemical approximation of milk, the cumulative consequence of that exposure over years is not negligible. It is simply invisible, because no single cup of tea causes a blood test to change. The damage is slow, systemic, and silent.
This is the part that food safety conversations usually leave out.
Why Mr. Mundhe's work matters beyond the headlines
The instinct when reading about raids and FIRs is to treat this as enforcement news. Important, yes. But filed somewhere separate from the kitchen, from the dinner table, from the daily glass of milk.
It should not be filed separately.
What Mr. Mundhe's operation has done is make visible a contamination that most people were consuming without knowing it. Nearly 40 percent of paneer samples tested by the Maharashtra FDA failed safety standards. Edible oil worth nearly three crore rupees was seized from a single unit in Chhatrapati Sambhajinagar, produced with rusted machinery and no quality control. The scale of what was reaching ordinary kitchens is significant.
What you can do
There is no perfect answer here, and the responsibility for food safety cannot be placed entirely on consumers. That is exactly what enforcement systems like the one Mr. Mundhe is now running are designed to address. But a few things are within reach.
Buying milk from cooperative dairies with verified testing protocols, such as those operating under state cooperative frameworks, reduces risk compared to unbranded or loosely regulated sources. Branded, packaged milk from FSSAI compliant dairies is not a guarantee but it is a better baseline than unverified loose milk.
Boiling milk before use does not neutralise chemical adulterants but does reduce the bacterial load significantly, particularly relevant when water contamination is a concern.
Supporting and paying attention to the enforcement work being done by institutions like the Maharashtra FDA matters at a civic level. Mr. Mundhe's campaign is producing results, but food safety enforcement in India is chronically under resourced. Public awareness creates accountability.
The bigger point
We spend a lot of time and effort thinking about what people eat. Macronutrients. Portions. Glycaemic index. Meal timing. All of it matters. But the quality and safety of the food itself, whether it is what it claims to be, is a layer underneath all of that, and it is one that most conversations on health quietly skip.
Mr. Mundhe's crackdown is not just an enforcement story. It is a reminder that the foundation of good health starts before the food reaches the plate.
